Leonora-Sage was founded by Marlene Nicholson. She received her Master of Social Work (MSW) from the University of North Carolina at Chapel Hill and is a Licensed Clinical Social Worker (LCSW) in NC and FL, a Certified Case Manager (CCM) and holds a graduate certificate in Healthcare Leadership. She brings over 16 years of dedicated service in medical and behavioral health, focusing on supporting seniors as well as individuals with complex medical and mental health needs.
Marlene is recognized as an Advanced Professional in the Aging Life Care Association® (ALCA), reflecting her adherence to the highest standards of practice. Her background also includes distinguished service as a retired veteran, having served in both the United States Army and the U.S. Public Health Service, further underscoring her lifelong dedication to service and care. Marlene’s background reflects a deep commitment to supporting those facing complex health challenges, and her leadership guides Leonora-Sage’s mission to empower families and honor the dignity of every client.
Aging Life Care™ is a holistic, client-centered approach that helps older adults and individuals with chronic illness or disability live safely and with dignity. Aging Life Care Professionals, trained in nursing, social work, gerontology, and related fields assess needs, create and monitor care plans, coordinate services, and advocate for clients as circumstances change. They serve as trusted guides for families, working closely with healthcare providers, elder law attorneys, wealth managers, and other professionals, especially in complex or crisis situations. An Aging Life Care Professional is a health and human services specialist who acts as a guide and advocate for families who are caring for older relatives or disabled adults. We are trained in any of several fields related to long-term care, including nursing, gerontology, social work, or psychology, with a specialized focus on issues related to aging.
While we often collaborate with those professionals, our role is holistic and clinical.
Home Health Aides provide direct, hands-on physical care.
Senior Move Managers focus on the logistics of downsizing.
Aging Life Care Professionals oversee the “big picture,” coordinating medical, legal, financial, and emotional needs to ensure a high quality of life.
Members of the Aging Life Care Association must meet stringent education, experience, and certification requirements. Most importantly, we are required to adhere to a strict Code of Ethics and Standards of Practice, ensuring that your loved one’s welfare is always the top priority.

The Evelyn McPherson Foundation has provided a grant that offers free Aging Life Care™ Management consultation to elderly and or disabled family members of active military members, veterans, or veteran’s elderly and or disabled family members (Family members are limited to a spouse, parent, or child). For eligible military families, the McPherson fund will provide up to 4 hours of an Aging Life Care Professional’s expertise; providing recommendations and referrals for the most appropriate service that fits the specific needs of the Veteran of family member. We do not provide hands-on care like bathing or medication administration. Instead, we vet, hire, and monitor the agencies or individuals who do. We act as your “eyes and ears,” ensuring that the care being paid for is actually being delivered at the highest standard.
Absolutely. This is one of our most common scenarios. We serve as the local professional representative for “long-distance caregivers,” providing you with regular updates and immediate crisis intervention if something goes wrong.

Services are primarily billed hourly, so you pay only for the time spent supporting you or your loved one. Most clients begin with an initial assessment, which may be billed at a higher rate or as a separate flat fee due to its comprehensive nature.
All service options will be discussed during your free consultation, and you’ll receive a clear written agreement outlining expectations, rates and scope before services begin to ensure everything is clear and aligned with your needs.
This is a very common challenge. We are trained in techniques to build rapport and de-escalate resistance. Often, a senior is more willing to listen to a “neutral professional” than a family member. We focus on autonomy and safety, helping them see how support can actually help them remain independent longer.
