  • Step 1 — Schedule a Complimentary Call

    Start with a free 15-minute call to explore whether Leonora-SAGE is the right fit for you and your family.

  • Step 2 — In-Home Initial Assessment

    When we're ready to move forward, we'll schedule a time for your Aging Life Care Manager ® to visit. Together we'll complete the service agreements and a thoughtful, collaborative assessment; at a pace that works for you.

  • Step 3 — Develop Your Individualized Care Plan

    This is often where families first feel a sense of relief. Based on the assessment, we'll create a personalized care plan built around your loved one's needs and your family's goals. From there, you choose how to move forward:

    📋 Take the Plan & Move Forward Independently You'll have a clear, actionable care plan to implement on your own terms and timeline.

    🤝 Continue with Ongoing Care Coordination Stay supported with your Aging Life Care Manager® providing continued guidance, monitoring, and advocacy every step of the way.

  • Step 4 — Ongoing Monitoring and Care Coordination

    Your care plan will be reviewed regularly and updated as needs change. Routine communication keeps you informed and involved. Over time, many families move from overwhelmed to reassured, knowing their loved one is in caring, capable hands.
