Your Leonora-SAGE, Aging Life Care Manager® will conduct comprehensive evaluations covering medical histories, cognitive function, daily living abilities, home safety, financial considerations, and emotional well-being. This includes early identification of memory loss, dementia, and cognitive decline that families may not yet have a name for. From this assessment, your Care Manager will create a personalized care plan with specific, actionable recommendations that are tailored to your family’s unique needs and goals.

When we're ready to move forward, we'll schedule a time for your Aging Life Care Manager ® to visit. Together we'll complete the service agreements and a thoughtful, collaborative assessment; at a pace that works for you.
