Clinical depth that goes beyond coordination.
Aging brings emotional and psychological challenges that standard care coordination was never designed to address (grief, anxiety, cognitive change, loss of identity, and the difficulty of major life transitions). For caregivers navigating a dementia diagnosis, this often includes the profound grief of watching an aging adult change, conflict between families over care decisions, and the emotional exhaustion of caregiving; all of which your Care Manager is clinically trained to address. As a Licensed Clinical Social Worker, we integrate SAMHSA’s Eight Dimensions of Wellness into care planning; addressing the whole person, not just the medical diagnosis. This is a whole-person wellbeing framework spanning physical, emotional, social, intellectual, financial, environmental, spiritual, and occupational wellness. This ensures understanding each client fully and designing psychosocial interventions that restores balance across their life.
This work is also guided by the SAGE model: providing genuine Support, ensuring each client’s needs are Advocated for within the care team, offering Guidance that families can act on with confidence, and building the tools and self-awareness that Empower lasting resilience.
Where deeper psychiatric or therapeutic care is needed, your Aging Life Care Manager’s® clinical background ensures referrals are targeted, informed, and made with a full understanding of each client’s history.
